Avivo Health Alliance is located at 2012 12 Ave NW.  

There is free street parking in front of the building on 12th Ave,  as well as on the side streets (20th street or 20a street NW).  PLEASE MAKE SURE YOU ARE NOT PARKED IN A RESTRICTED ZONE.  Expect your appointment to last approximately 1.5 hours.  Unfortunately if you are late, we may need to reschedule your appointment.
